Proposal:
The Proposal file is loaded as one PDF file, and usually includes the Title Referral Page, Table of Contents/Checklist, Proposal Relevance Statement, Main Body (technical narrative), List of Abbreviations/Acronyms, References, Biographical Sketches, Existing/Pending Support, Facilities/Equipment Description, Questionnaires/Surveys (if applicable), Publications and/or Patent Abstracts, and Administrative Documentation.

This list is not exhaustive, nor does it represent the requirements of all CDMRP programs. Please review your program announcement for specific program requirements and guidelines for each section of the "Proposal."

Budget Information:
Budget Information is loaded as one PDF file, and includes the Detailed Cost Estimate and Budget Justification. Generic budget forms can be found at https://cdmrp.org/programAnnouncements.cfm. Please review your program announcement for budget requirements and/or limitations.

Please route your budget to Sponsored Projects via fax before uploading the budget PDF file.  Sponsored Projects will advise you of necessary corrections to include in the final upload.

Certification of Environmental Compliance:
This form must be signed, scanned, and uploaded as a PDF file. The form can be found under the link above. It can be signed when the Proposal Routing Sheet is routed through Sponsored Projects, or by Risk Management.

Principal Investigator Assurance of the Safety Plan:
This form must be signed by the Principal Investigator and uploaded as a PDF file.

After all required files are uploaded, the File Upload process must be completed by by clicking the "Finalize Required Files for CR Approval" button.  Submit the File Upload page as FINAL to allow the submission process to continue.
